I had experienced similar illogical claims denials and it was only after spending hours on the phone with a customer service rep from Anthem BC BS that I learned as of mid 2024, Anthem switched to an AI claims processor that required a specific format for the bill in order for the AI to read it appropriately. What baffles me most is the format is not the standard HCFA CMS-1500 form that is standardized by Medicare/Medicaid.

Regardless, I reformatted my bills, recommended patients do the same and the denial rate returned to the pre-AI rollout level. (Still frustratingly high but better)…

Sounds like an implementation problem with the AI system. If there is a standard for these bills as determined by CMS and the insurance companies are not compliant, and systems deny claims, and this result in injury, I would assume this is grounds for claims against the insurance company. A good team of attorneys should be able to drive these insurance companies into bankruptcy.
